East Bay Veterinary Hospital requires PAYMENT AT TIME OF SERVICE RENDERED and does not do billing. If payment is not received upon services rendered, a monthly billing fee of $5.00 and an interest fee of 1.5% per month compounded monthly for an effective annual rate of 18% will be charged. If any unpaid bill is sent to collection, it will be subject to an additional 30% collection fee. I hereby authorize East Bay Veterinary Hospital to debit my credit card on file. Upon signing this form, I agree to allow East Bay Veterinary Hospital employees to provide treatments and medications as deemed necessary and understand that certain approved drugs may be used with “off label” applications.
